Dink’n in the Hills Pickleball Tournament
Waiver, Release of Liability, Photo/Media Release, and Refund Policy
By submitting this form, I acknowledge that I am registering for and/or participating in the Dink’n in the Hills Pickleball Tournament hosted by Canyon Lake Activity Center. If I am submitting this form for a minor participant, I confirm that I am the participant’s parent or legal guardian and have authority to agree to this waiver on the minor participant’s behalf.
I understand that pickleball and related tournament activities involve risks, including but not limited to slips, falls, collisions, muscle strains, sprains, broken bones, heat-related illness, contact with paddles or balls, court conditions, weather conditions, and the actions of other players, spectators, volunteers, or staff. I voluntarily choose to participate, or allow the minor participant to participate, and accept full responsibility for that participation.
To the fullest extent permitted by law, I release, waive, discharge, and hold harmless Canyon Lake Activity Center, Canyon Lake Pickleball, tournament directors, organizers, board members, employees, volunteers, sponsors, photographers, representatives, and all others associated with the tournament from any and all claims, injuries, damages, losses, expenses, or liability arising out of or related to my registration, participation, attendance, or involvement in the tournament, or the minor participant’s registration, participation, attendance, or involvement in the tournament.
I understand that tournament schedules, divisions, formats, locations, start times, and rules may be changed as needed. Divisions may be combined, split, modified, or canceled based on registration numbers, weather, facility needs, safety concerns, or other tournament considerations.
I grant Canyon Lake Activity Center permission to photograph, film, record, publish, and use my name, image, likeness, voice, and/or tournament results, or the minor participant’s name, image, likeness, voice, and/or tournament results, for Canyon Lake Activity Center’s website, social media, newsletter, printed materials, promotional materials, fundraising materials, advertising, and other public communications. I understand that I will not receive compensation for these uses.
Refund Policy: I understand and agree that all sales are final and there are no refunds.
I agree to follow all tournament rules, facility rules, safety instructions, and directions from tournament officials. I understand that Canyon Lake Activity Center and tournament organizers may remove any participant or spectator for unsafe, disruptive, unsportsmanlike, or inappropriate behavior without refund.
By submitting this form, checking the required agreement box, and/or providing my electronic signature, I confirm that I have read, understand, and agree to this waiver, release of liability, photo/media release, and refund policy for myself and/or the minor participant. I understand that my electronic signature and/or form submission is intended to have the same effect as my handwritten signature.
